How are Red Ache and Soldier Shakes working on Amia?

There's been no changes to how diseases work on Amia, to my knowledge.

Ah, but do they need changes now that I think on it. Diseases and poison are in that "really need beefing" group perhaps?

Soldier shakes is resisted upon contraction if the target makes a successful initial fortitude saving throw. After the incubation period, subsequent fortitude saves are attempted to resist the ability damage. Two successful subsequent saves in a row will remove the disease (but not the side-effects). After 24 hours, the diseased target will have one of their ability scores permanently lowered by 1.

The permanent reduction isn't on Amia. To my knowledge NWN doesn't support that. A restoration or similar will still fix you right up.

"permanent" simply means "until cured", not a physical stat reduction.

Possibly, though I see them as a more RP flavour thing now. Something that afflicts commoners and NPCs, not mighty heroes.
