The spell name that is casted from this item is "Negative Plane Avatar" and supposed to turn user into that avatar. However, for some unknown reason it turns user into water elemental.
I suppose it is kind of bug in script, and since almost noone uses it, it was that way for long time already.
It would be nice if that was fixed, somehow.

It always worked for me. Vrock. I think it alignment based though, what alignment are you?

Edit: Actually, let me guess. You are a female neutral character? If yes, that's intended, if weird, behaviour. It's the spell from the deck of hazards item called "the avatar". It transforms you into an "avator of [your] chosen alignment". Apparently, water elementals are very typical for female neutral people...

I'd imagine the description factors in that it only usable by evil, in which case it gets you a vrock. Or a succubus? Seems a bit weird, but it almost works. But if it used by a non-evil it gets even weirder, since it's kind of not intended behaviour.

Good gets you a celestial avenger. You used an evil item. You suddenly turned into crazy angel doge.

Huh, I wasn't even aware we had an item with that spell on it ingame. And yeah its a spell from a Deck of Hazards item meant to transform you into your alignment paragon, which is nice and all if you are a low level PC in the basegame but will noticeable make you weaker on Amia if I remember the stats correctly.

Well, I know that the vrock is kind of niche tranformation, but at low-levels, on a sneaker, I used to take that before I got any good weapons. They get pretty good stats for low-level hunting, even though it loses its uses after like level 10. And it's nice for trolling people. Sneak up to them, unsneak and *screech*. Since people don't know it outs you as evil, they won't metagame you being evil, and instead think something along the lines of "what is going on?".
