Hallv's axe, while unequipped regestured as equipped on my screen, though the item itself appeared as if it would be an item on the ground with various options. Equipping and unequipping the weapon fixed the problem.

I'm thinking this was a one off glitch, but I will leave this open for a month to see if anyone else has similar issues.

It happens sometimes. Like when you equip/unequip too fast and you are still visually holding a weapon/shield even though it's not in your inventory slots. Silly NWN.
