The question has came up so lets answer a few questions on it. Alignment is a terrible thing that means very little without understanding the moral context of it. Most people's views of alignment are colored by their own values, likes, dislikes and what not. This is why most religions had decided that judgement on this level was left for the gods, because of the incapability of mortals like ourselves to accurately describe it. However, lets give it a try.

This will be updated as new ideas are proposed and provided.

The Law and Chaos Spectrum

Lawful
Lawful characters are those who love structure and "judge" the world with the perceptions of the whole of the group. Self is valued little in these groups and conformity is a must, for survival usually. In many eastern religions this was associated with the drive to structure and quantify things. It was associated with death because hyper order would destroy the freedoms required to reform the society.

Planes of Law: Celestia, Arcadia, Mechanus, Archeon, Baator

Chaotic
Chaotic characters are those who want to explore and experience things rather than structure and judge. Often wistful and scatterbrained, they are often the best on their feet and make a many of great artists and explorers. Where a lawful character would go out on an adventure to ensure the structure of his or society is protected, a chaotic seeks out what is on the fringes of those boundaries to make it better. Chaos is the force that goes with the flow of things and cares little for rules and regulation. They choose a path of pure freedom to experience all the world gives them, which then they then can reform the society with fads and what not.

Planes of Chaos: Arboria, Ysgard, Limbo, Pandemonium, Abyss

The Good and Evil Spectrum

Good
Good is the act of helping others and will seek to empathize with others and protect them when need arises. On an extreme they are martyrs. In Christianity goodness is represented by the Seven Heavenly Virtues; Humility, Kindness, Forgiveness, Zeal (for Work), Generosity, Moderation, and Purity.

Planes of Good: Celestia, Bytopia, Elysium, Beastlands, Arborea

Evil
Evil is selfish action which may or may not harm others. They are apathetic of the suffering of the others. At its extreme it can represent the enjoyment of causing suffering. In religions, this is the manifestation of the "balckest" part of the person's being which their darkest emotions recide. In Christianity goodness is represented by the Seven Deadly Sins; Pride, Envy, Wrath, Sloth (Or Laziness), Greed, Gluttony, and Lust.

Planes of Evil: Baator, Gehenna, Hades, Carceri, Abyss

any post that isn't the PHB quotes about alignment with "take this with a huge grain of salt because you can argue about this forever" is completely irrelevant and doesn't deserve to be in the lore forum, something meant for hard, concrete answers to question.

Take this with as much salt as you want, but my own personal interpretation of the law and chaos spectrum is tied to the Big Fine personality traits. It is my belief that a characters placement on the Law and Chaos spectrum is tied to their agreeableness trait, which reflects individual differences in general concern for social harmony. Meaning people with high agreeableness are generally more lawful, and people low in agreeableness are generally more chaotic in nature.

As Tormak said, take whatever isn't taken directly from the sourcebooks with a huge grain of salt. For the record, this is what the source material says on the subject.
